  2. Practice Information – NPI, Tax ID, and provider details

  3. System Access – EHR, clearinghouse, and payer portals

  4. Fee Schedule & Coding Preferences – Your preferred codes and encounter forms

  5. Credentialing (if needed) – CAQH, licenses, and payer enrollment details

  6. Accounts Receivable (optional) – Aging report if you’d like us to help with unpaid claims
